Caregivers are connected directly with payers and care coordinators to report real-time care logs and patient updates. This critical caregiver data is integrated with health plans, hospitals and aco’s technology platforms to delivered a comprehensive digital care plan that better supports patients receiving care at home.

Care Heroes is a caregiver engagement platform that enables managed medicaid and medicare health plans, dual eligible payers, accountable care organizations, home health companies and other service organizations to track, reimburse, recognize and reward home health caregivers (professional, unpaid and family members).

Caregiver Task Reporting & Validation: The caregiver engagement platform connects directly with the subcontracted, paid caregivers delivering care to their members. Our web and mobile products enable the assigned caregiver to securely track and report daily care plan tasks that are completed back to the to the health plan and the responsible care plan managers
Our GPS monitoring capabilities, validates that care had been delivered to the approved health plan member. The enables the health plan to follow the covered member throughout the continuum of care from acute settings and back into the home.
Provider Case Manager Dashboard: Health plan case managers can access Care 1 Heroes caregiver reporting platform in real time and review caregiving tasks completed by paid caregivers delivering care to their members. A secure messaging/chat platform also enables a care managers to easily communicate with identified caregivers and provided much needed support and advice.
